The deadline for submitting a request to have the GMAT required of you be waived varies by institution. Just do as they say. Unless otherwise stated, waiver requests should be sent in writing to the admissions committee. It is also wise to start communicating with them early in the process, in case it takes some time for them to approve your unique request.
In the absence of instructions to the contrary, you should write your GMAT Waiver Request Letter using standard business letter format. Address, date, subject, and salutations should all be formatted similarly to other formal letters sent in connection with applying for admission (Letter of Intent vs Statement of Purpose vs Personal Statement).
In terms of the essay's actual content, you'll want to make sure you give a convincing justification for why you don't need to take the Graduate Management Admissions Test (GMAT). Most schools have specific eligibility requirements for the bonus funding. Maintain a level of modesty and avoid making your claim sound too good to be true; instead, focus on the essentials.
Here is an example, albeit completely fictitious, GMAT waiver letter from a candidate with a Master of Science in a quantitative field and relevant work experience. Attention, Admissions Board

Full-time MBA Program Application - GMAT Waiver Request
To Whom It May Concern,
Just state the gist of your argument in the first paragraph without going into too much depth.
Please accept this letter as my formal request to have the GMAT requirement for admission to your Full-Time MBA programme waived. In light of my prior experience and education, I believe I have the necessary qualifications to be considered for a waiver.
Explain in detail your academic and professional accomplishments that align with the criteria the school requires for a waiver in the second paragraph.
I earned my Master of Science in Economics from XYZ University (which is, of course, legitimate) with a 3.5 grade point average. My career path led me to EFG Corp. as a Financial Risk Analyst after I graduated in 2010 and completed a two-month internship at ABC Company during my undergraduate studies. My duties in this position entailed (please list your responsibilities here with appropriate restraint). You should avoid coming off as overly complimentary. It took me 2 years, but I finally made it through and earned my IRM (Institute for Risk Management) certification. Soon after, I was promoted to Risk Manager of my division, where I now oversee ten people.
In the third paragraph, please demonstrate your abilities and the results you've achieved. Tell us about the difficulties you encountered and how your arithmetic prowess came to the rescue.
Fourth Paragraph: Thank the Adcom for their time and kindly ask that your qualifications for an exemption from taking the GMAT be carefully considered.
Please accept my sincere regards,
Aman Jha
